Nurse Practitioner (NP) – Wound Care (Travel, Long-Term Care Facilities)

Position Summary:

We are seeking an experienced and compassionate Nurse Practitioner to provide advanced wound care services to residents in long-term care and skilled nursing facilities. This role involves traveling between assigned facilities to assess, diagnose, and treat acute and chronic wounds, ensuring high-quality, patient-centered care while collaborating with facility staff and physicians.

Key Responsibilities:

  • Perform comprehensive wound assessments, develop individualized treatment plans, and provide advanced wound care interventions.

  • Conduct regular wound rounds and monitor healing progress, adjusting care plans as needed.

  • Educate facility staff and caregivers on wound prevention, treatment protocols, and best practices.

  • Collaborate with interdisciplinary teams including physicians, nurses, therapists, and dietitians to optimize outcomes.

  • Document all patient encounters, treatment plans, and progress in accordance with regulatory and company standards.

  • Travel between multiple long-term care facilities within the assigned region to provide consistent coverage and care.

  • Serve as a clinical resource for wound care management, infection prevention, and skin integrity maintenance.

Qualifications:

  • Current and unrestricted Nurse Practitioner (NP) license in the state of practice.

  • Board certification as an NP (FNP, AGNP, or similar).

  • Minimum of 1–2 years of wound care experience in long-term care, home health, or acute care (WOCN certification preferred but not required).

  • Strong knowledge of wound care modalities, including debridement, negative pressure wound therapy, and infection management.

  • Excellent clinical, documentation, and communication skills.

  • Ability and willingness to travel between facilities; reliable transportation required.
